There are two ways to apply for HMK:
- Download the application form, print it out, fill it in by hand, sign the application, and mail or fax the application to the address/fax number on the application. Be sure to include income and citizenship documentation.
- Enter the information directly into the application on your computer, print it out, sign the application, and mail or fax it to the address/fax number on the application. Be sure to include income and citizenship documentation.
